Knee Pain Physical Therapy in Ellicott City, MD
Knees keep score. Every staircase, every squat to the bottom shelf, every mile and every shift on your feet gets tallied, and when a knee starts complaining, it rarely stops on its own. Here is what most people are not told: knee pain very often starts somewhere else, in how the hip controls the leg or how the foot meets the ground. At Revive Physical Therapy, you work one-on-one with a licensed physical therapist who evaluates the whole chain, not just the joint that hurts. With appointments within 24 hours, weekend hours, and two Ellicott City locations, answers are a short drive away.

Who Our Knee Pain Program is For
Our knee pain program serves adults and athletes across Ellicott City and Howard County whose knees have started making decisions for them: runners and recreational athletes with pain that builds through activity, workers whose jobs demand kneeling, squatting, and stairs, older adults managing arthritis while trying to stay active, and patients recovering after knee surgery.
The knee sits between the hip and the foot, and it often pays the bill for problems in either. A hip that does not control the leg’s alignment, or a foot that collapses inward with every step, changes where load lands in the knee thousands of times a day. That is why treatment aimed only at the painful spot so often disappoints, and why our evaluation looks at how your whole leg works. Whatever brought your knee here, sudden injury or slow wear, the path forward starts with understanding what is actually driving the pain.
Active adults and athletes
Care that keeps you training and playing while the knee recovers, with a clear plan back to full activity.
Workers on stairs, ladders, and floors
Treatment built around jobs that cannot stop kneeling and squatting while the knee gets better.
Adults with knee arthritis
A structured strengthening approach for staying active with arthritis, including knees labeled “bone-on-bone.”
Knee Conditions We Treat
Knee pain comes in recognizable patterns, and the pattern points toward the source. These are the knee conditions our physical therapists treat most often at our two Ellicott City clinics:

How Revive Evaluates Knee Pain
Your first visit is a full one-on-one evaluation with a licensed physical therapist, typically lasting 45 minutes to an hour. We start with your history: how the pain began, exactly which activities and positions provoke it, any swelling, catching, or giving way, and what your knees need to handle for your work, sport, and life.
Then we watch you move. Rather than only treating the painful spot, we evaluate how you move to identify the root cause of the problem. For knees, that means watching the whole leg work: how your knee tracks during a single-leg squat, what happens to your alignment coming down stairs, how you land, and whether your hip is controlling the leg or letting the knee drift inward under load. Those patterns, repeated thousands of times a day, often explain why a knee hurts far better than any single structure does, and they are only visible when a therapist watches you do the movements that hurt.
Single-leg movement analysis
Single-leg squat, step-down, and landing mechanics assessed for the alignment patterns driving knee load.
Hip and foot contribution
How the joints above and below are shaping what your knee has to absorb with every step.
Strength testing
Objective measures of quad, hip, and leg strength so progress is tracked with numbers, not guesswork.
Activity and load review
Your training, work demands, and recent changes, because spikes in load precede most knee flare-ups.
What Knee Pain Treatment Includes
Every plan is individualized around one goal: a knee that handles your full life again, not one you have learned to protect. Resting a painful knee usually trades pain for weakness, and a weaker leg puts more stress through the knee, not less, which is how short problems become long ones. Instead, we modify how you load the knee so daily life, work, and as much activity as safely possible continue while the irritated tissue settles.
Your program typically combines hands-on manual therapy for the stiffness around the knee, hip, and ankle, progressive strengthening for the quads and hips, the two engines that protect a knee, and retraining of the squatting, stair, and landing mechanics that were concentrating load on the painful spot. Our therapists hold advanced certifications in orthopedic manual therapy and dry needling, which can be incorporated into your plan when appropriate. As you improve, we build a graded return to your full demands, from stairs and work tasks through running, jumping, and sport.
You also leave every visit with a focused home program, because the strength that protects knees is built between sessions, not only during them.
Manual therapy
Hands-on treatment for the knee, hip, and ankle restrictions limiting comfortable movement.
Quad and hip strengthening
Progressive loading for the muscles that absorb force before it reaches the joint.
Movement retraining
Correcting the squat, stair, and landing patterns that keep overloading the same spot.
Graded return to demands
Clear criteria for adding back stairs without thought, full workdays, running, and sport.
Why One-on-One Care Matters for Knee Pain
At many large physical therapy clinics, your therapist may divide attention between multiple patients, with part of the visit spent completing exercises independently. Knees expose that shortcut quickly. The inward drift of a knee during a squat or step-down is subtle, it worsens exactly when you fatigue, and left uncorrected it means every strengthening rep rehearses the same faulty pattern that caused the pain. Correcting it takes a therapist who is watching your alignment on every repetition, not checking in between patients.

Knee Pain Physical Therapy: Frequently Asked Questions
Do I need a doctor’s referral to start physical therapy?
As a patient, you have the right to choose your physical therapy provider, and in most cases you can schedule an evaluation with us directly. Some insurance plans do require a physician referral or prescription for coverage, so call us at (410) 988-5171 or (443) 364-3015 and our team will verify your benefits and let you know exactly what your plan requires.
Do I need an MRI before starting physical therapy for knee pain?
Usually not. A thorough clinical evaluation identifies the likely source of most knee pain and is the standard starting point. Knee MRIs also carry a known caveat: meniscus and cartilage changes show up frequently in adults with no knee pain at all, especially with age, so a finding on imaging does not automatically explain your symptoms or dictate your treatment. If your evaluation or your progress suggests imaging would genuinely change the plan, we will tell you and coordinate with your physician.
Can physical therapy help a “bone-on-bone” knee?
Very often, yes, and this surprises people who heard “bone-on-bone” as the end of the road. Exercise-based treatment is a first-line, guideline-recommended approach for knee arthritis, and building the strength of the muscles around the knee reliably reduces pain and improves function for many people, whatever the X-ray shows. For some, that means delaying or avoiding a replacement; for others, it means going into an eventual surgery stronger and recovering faster. Where your knee lands on that spectrum is a decision for you and your physician, and a structured trial of strengthening gives that decision real information.
Is it safe to strengthen a knee that hurts?
With the right dose, yes, and it is usually essential. Hurt does not automatically equal harm: some manageable discomfort during and after rehab exercise is common and acceptable within limits your therapist sets with you, and avoiding all loading tends to leave the knee weaker and more painful over time. The skill is in the dosing, choosing the right exercises at the right intensity for where your knee is now, and progressing as it adapts. That calibration is exactly what your therapist manages visit to visit.
What should I expect at the first visit, and how long will recovery take?
Your first visit usually lasts 45 minutes to an hour and includes a full one-on-one evaluation. Recovery time depends on the condition, how long it has been present, and the demands you are returning to. After the evaluation, your therapist will discuss your recommended number of sessions and give you a realistic timeline with specific milestones, such as when to expect comfortable stairs and a return to running or full workdays.
Does insurance cover physical therapy for knee pain?
Coverage varies by plan. Revive Physical Therapy accepts all major insurance plans, and our team can verify your benefits before your first visit. Call either location or visit our Insurances page for more information, and see our Self Pay options if you are paying out of pocket.
Have a question we did not cover here? Our What to Expect page answers common questions about insurance, what to wear, and what to bring to your first appointment.
