Your Insurance Is Welcome Here
We work with a wide range of insurance plans — including auto injury and self-pay options — so cost is never the reason you put off getting better.
Insurance Shouldn’t Stand Between You and Your Recovery
At Revive Physical Therapy, we believe that navigating insurance shouldn’t feel like a second job. That’s why we accept a broad network of insurance providers and work directly with your plan to verify coverage so you can walk in focused on healing, not paperwork.
Physical therapy is one of the most cost-effective investments in your long-term health. Most insurance plans — including Medicare, Medicaid, and major private providers — cover PT for a wide range of conditions. Not sure what your plan covers? We verify your benefits before your first visit so you walk in knowing exactly what to expect, with no surprises on your bill.
Insurance Plans We Work With
Most major commercial plans are in-network at both our Baltimore National Pike and Waterloo Road locations. If you do not see your plan listed, call us — eligibility verification is always free and takes just a few minutes.
Don’t see your plan? Call us — we verify benefits at no cost before your first visit and work with many plans not listed here. We will tell you exactly what is covered before you commit to anything.
How Insurance Works at Revive
Three straightforward steps — and we handle the majority of the work so you can stay focused on getting better.
Book Your Appointment
Schedule online or call either of our clinics. Let us know your insurance provider and we’ll take it from there.
We Verify Your Benefits
Our team contacts your insurance provider directly before your first visit to confirm your coverage and any out-of-pocket costs. We handle this for you.
Come In and Start Healing
Walk in knowing exactly what’s covered. No surprises, no insurance stress — just focused, expert care from day one.
Injured in a Car Accident? Your PT May Be Covered
If you were injured in a car accident, your auto policy may cover physical therapy through Personal Injury Protection (PIP). Our team will verify your auto insurance benefits directly so you know what’s covered before your first visit.
Work Injury? We Accept Workers’ Compensation
If you were injured on the job, your physical therapy may be covered under Workers’ Compensation. Our team works directly with your employer’s insurer to verify your coverage and handle authorization so you can focus on your recovery.
Don’t Have Insurance? Self-Pay Options Are Available.
We believe cost should never be a barrier to getting the care you need. Revive Physical Therapy offers competitive self-pay rates for patients without insurance coverage — ask us about our current rates when you call.
Insurance FAQ
Plain answers to the questions patients ask most — no legal language, no runaround.
No — Maryland is a direct access state, which means you can begin physical therapy without a physician’s referral. You can call us today, schedule your evaluation, and start treatment without needing a separate doctor’s appointment first. Some insurance plans still require a referral for billing purposes, which is something we verify on your behalf during our benefits check before your first visit — so you will know in advance if one is needed.
You do not need to call your insurance company yourself — we handle that for you. Before your first visit, our billing team contacts your insurer directly to confirm whether PT is covered under your plan, how many visits are authorized, what your copay or coinsurance will be, and whether a deductible applies. We give you a clear, plain-language summary before you step through the door. No guesswork, no surprises after the fact.
A copay is a fixed amount your plan requires you to pay at each visit — for specialist visits this typically falls between $20 and $60, though it varies by plan. Not every plan uses copays; some apply visits toward an annual deductible or use coinsurance (a percentage of the visit cost) instead. We confirm the exact amount for your specific plan during benefits verification, so you will know your out-of-pocket responsibility before your first appointment, not after.
Yes — we accept both Medicare and Medicaid at both of our Ellicott City locations. Medicare typically covers outpatient physical therapy when treatment is medically necessary. Medicaid coverage varies by managed care plan, and our team verifies your specific benefits before your first visit. If you have a Medicare Advantage or Medicaid managed care plan, we check those as well and confirm your coverage details in advance so there are no surprises.
Claim denials happen, but they are rarely the final word. Our billing team reviews every denial, identifies the reason, and files an appeal on your behalf when the denial appears incorrect or disputable. Many denials are successfully reversed through the appeals process — and we manage that entire process without placing any burden on you. If a denial stands and results in a balance owed, we contact you directly to discuss your options before anything further happens. You will never be blindsided by a bill.
Yes — physical therapy is an IRS-qualified medical expense, which means you can use Health Savings Account (HSA) or Flexible Spending Account (FSA) funds to cover copays, deductibles, coinsurance amounts, or self-pay session costs. Simply use your HSA or FSA card at time of payment, or pay out of pocket and submit your receipt to your plan administrator for reimbursement. Your plan administrator can confirm exactly what qualifies under your specific account.
