To help us prepare for your visit, please bring the following:
- Your physical therapy prescription (if required by your provider)
- Your insurance card
- Completed digital intake form (you’ll receive this via email after scheduling)
- If your visit is related to an auto accident or worker’s comp case, bring your claim number and adjuster contact information
- Comfortable clothing that allows access to the area being treated
- Please arrive 15 minutes early to complete any remaining paperwork
